On May 31, 2016, Progressive Acute Care, LLC and certain of its affiliates (collectively, the "Debtors”) filed voluntary petitions for relief under Chapter 11 of title 11 of the United States Code (the "Chapter 11 Cases") in the United States Bankruptcy Court for the Western District of Louisiana (the "Bankruptcy Court"). These Chapter 11 Cases are being jointly administered under the main case, 16-50740. The cases have been assigned to the Honorable Chief Judge Robert Summerhays.

On March 14, 2017, the Debtors filed their Joint Plan of Liquidation (Docket No. 465) (the “Joint Plan”) and accompanying Disclosure Statement (the “Disclosure Statement”). An Amended Disclosure Statement was filed on April 18, 2017 (Docket No. 504).

On July 12, 2017, the Bankruptcy Court entered the Findings of Fact, Conclusions of Law and Order Confirming Joint Plan of Orderly Liquidation of Progressive Acute Care, LLC, et al., as Jointly Proposed by the Debtors and the Official Committee of Unsecured Creditors (Docket No. 550).

August 1, 2017 was the Effective Date of the Joint Plan (Docket No. 558).
