On January 30, 2017, LMCHH PCP LLC and certain of its affiliates (collectively, the "Debtors”) filed voluntary petitions for relief under Chapter 11 of title 11 of the United States Code (the "Chapter 11 Cases") in the United States Bankruptcy Court for the District of Delaware (the "Bankruptcy Court"). On February 14, 2017, these cases were transferred to the United States Bankruptcy Court Eastern District of Louisiana. The cases have been assigned to the Honorable Judge Jerry A. Brown and are being jointly administered under the main case 17-10353.

On May 17, 2017, the Bankruptcy Court established July 10, 2017 at 5:00 p.m. (CT) as the deadline to file General Proofs of Claim and August 10, 2017 at 5:00 p.m. (CT) as the deadline to file Governmental Proofs of Claim. On June 23, 2017, the Bankruptcy Court also established July 28, 2017 as the last date and deadline by which Administrative and Postpetition Medical Malpractice Proofs of Claim must be filed. On June 29, 2017 the Debtors filed their Joint Plan of Liquidation (Docket No. 416) (the "Joint Plan") and accompanying Disclosure Statement (Docket No. 417). An Amended Joint Plan (Docket No. 470) was filed on August 1, 2017. On August 4, 2017 an Order approving the Disclosure Statement, establishing procedures for solicitation, and establishing the date for the Confirmation Hearing (Docket No. 484) was entered. A Confirmation Hearing was held on September 25, 2017 at 9:00 a.m. (CT). The Honorable Judge Brown confirmed the plan at the confirmation hearing and an order was entered (Docket No. 566) on October 2, 2017. The Plan became Effective on November 1, 2017 (Docket No. 608).
